Support to keep living well at home, on your own terms.

Most of us would like to stay in our own home as we get older. Support at Home is the Australian Government’s in-home aged care program, and it’s one of the ways we support older Territorians to keep living well at home. Whether you’re looking for support yourself, or helping an older parent, partner or friend work out what comes next, we’re here to make it simpler.

TeamHEALTH’s Support at Home services are available in Darwin and Palmerston.

What is Support at Home?

Support at Home is the national aged care program that helps older people get the support they need to keep living safely and independently. It replaced Home Care Packages and Short-Term Restorative Care in November 2025, bringing in-home aged care together under one system.

Through the program, people choose the services that suit them, how they’re delivered, and the level of support that fits their life.

Our Approach

Our services are flexible, clear and built around your needs. With clear pricing and no hidden fees, there are no surprises, just the confidence to plan support and make the most of Support at Home funding.

What We Offer

Support is shaped around each person, and can include:

  • Help around the home – cleaning, laundry, meal preparation, and home maintenance
  • Personal care – assistance with showering, grooming, and daily routines
  • Social and community support – companionship and support to keep up the social side of life
  • Getting out and about – transport to appointments, shopping, and social outings
  • Respite – a break for carers, knowing the person they care for is in good hands
  • Care planning – working alongside each person to get the most from their funding

Who it’s for

Support at Home is for older people who need some support to keep living safely and independently at home. Generally, that means someone who is:

  • Aged 65 or over, or
  • Aged 50 or over and Aboriginal and/or Torres Strait Islander

Clear and Simple Pricing

Our pricing is all-inclusive, meaning there are no hidden costs, just one clear rate for each service you receive.

  • When a contractor delivers a service on our behalf, a 10% administration fee applies.
  • We review our fees regularly to keep them fair and current. If anything changes, we’ll give at least four weeks’ notice.
  • Some services have a minimum booking time for example one to two hours; which we’ll confirm before booking.
  • If a contribution applies, Services Australia will be in touch directly.
  • Cancellations made with less than 24 hours’ notice are charged at the full cost of the service.

Making a referral

The first step is an aged care assessment through My Aged Care. You can request one online or by phone. Once approved, you will receive a referral code and can choose TeamHEALTH as your provider.

If you’d like a hand understanding any of this, get in touch, we’re happy to talk it through. We accept referrals from My Aged Care, health professionals, and directly from older people and their families and carers.
