Home Wellbeing Program
The Home Wellbeing Program supports older people whose home has become hard to manage, in a way that is affecting their wellbeing, independence, health or safety.
That might look like a build-up of clutter, finding it hard to sort through or part with belongings, or struggling to keep on top of the home and anything about the space that’s making everyday life harder.
These things rarely happen on their own. Grief and loss, trauma, changes in mental or physical health, getting around less easily, isolation, or a big change in life can all shape how someone manages their home, and how they feel about changing anything.
Our work is about much more than the home itself. We take the time to understand what’s going on, work out what matters most, and ease the sense of overwhelm, building confidence and everyday skills, strengthening connections and supporting changes that last.
Practical help around the home can be part of that, but it’s always guided by the person’s own goals, one piece of a wider plan built around their wellbeing, not just their home.
TeamHEALTH’s Home Wellbeing Program is available across Darwin and Palmerston.
The Home Wellbeing Program is delivered by TeamHEALTH through the Australian Government's Commonwealth Home Support Programme (CHSP).
Our approach
Change tends to last when people feel safe, respected, understood, and part of the decisions that affect them. That’s where we start. Our support is:
- Understanding what matters – recognising how trauma, loss and hard life experiences can shape how someone feels about their home, their belongings, and trusting others to help.
- Building on strengths - starting from what’s already working: the skills, routines, relationships and past successes a person can build on.
- Person-led – this is about choice, hope, and each person leading their own way forward, at their own pace.
- Working together – making practical, achievable changes that reduce risks to health, safety and independence, knowing change can happen gradually.
- Confidence that lasts – building the routines, strategies and problem-solving that help a person manage well beyond our involvement.
- Connection and coordination – easing isolation and linking in the right supports, from health and aged care to housing, family and community.
What we offer
Support is shaped around each person, and can include:
- A space to talk things through – support with the wellbeing side of things: overwhelm, grief, loss, isolation or life changes that are affecting everyday life.
- Building confidence and motivation – working out what matters, what feels doable, and moving towards goals at a comfortable pace.
- Everyday skills and routines – practical strategies for organising, deciding and keeping the home manageable day to day.
- A safer, easier home – changes that improve access, comfort, and the ability to use the rooms that matter most.
- Sorting belongings and spaces – where it’s a priority, working through things gradually and respectfully, with the person always in control.
- Connecting with the right supports – linking in health, aged care, housing and community services, and the people already in someone’s life.
- Making it last – reviewing what’s working and building strategies to hold onto progress after our supports winds down.
We can also arrange a cleaning or specialist services where they’re needed. Cleaning on its own doesn’t get to the heart of what’s going on and is not the focus of this program.
Who it’s for
The Home Wellbeing Program is for older people who’d like some support to keep their home safe and comfortable to live in. Generally, that means someone who is:
- aged 65 or over, or
- aged 50 or over and Aboriginal and/or Torres Strait Islander

To access the Home Wellbeing Program, the first step is an aged care assessment through My Aged Care, you can request one online or by phone. Once approved, the person receives a referral code and can choose TeamHEALTH as their provider. We're happy to help you make sense of any part of this along the way.
