Coordinator, Child and Family Wellbeing Service - Katherine

Position Type: Full Time

Location: Katherine

Reporting to and working closely with the Team Leader, the Coordinator will assist in the development, implementation and delivery of the Child and Family Wellbeing Service in the Katherine and Big Rivers region. They will be required to deliver strength based case management in line with program guidelines.

To facilitate the development of strong community relationships and support for participants of the Program the Coordinator will work with the Team Leader to develop collaborative partnerships and formal links with other services in the Katherine and Big Rivers region. They will assist in establishing good referral pathways, into and out of the service in order to reach vulnerable children, young people, families and carers, who may not otherwise engage with the mental health or children's services sector.

Mental Health Recovery Worker - Katherine

Position Type: Full time

Location: Katherine

Reporting to the Team Leader, Rural and Remote, the Mental Health Recovery Worker provides outreach support to participants using the psychosocial rehabilitation framework, with a focus on supports that are person centred, recovery focused, strengths based, timely and culturally appropriate.

The Mental Health Recovery Worker will develop, in consultation with the participant and where appropriate, other key stakeholders, an Individual Recovery Plan (IRP) to assist in achieving personal goals and positive outcomes for mental health, physical health and wellbeing. Supports provided will assist participants to maintain and increase their independence and live valued lives within the community.

Senior Child and Family Wellbeing Practitioner - Katherine

Reporting to the Team Leader, the Senior Child and Family Wellbeing Practitioner will work closely with the Youth and Family Services Manager at Kalano to focus on delivering early intervention, risk assessment, risk minimisation and relapse prevention approaches for participants referred to the CaFIS Program which is both timely and culturally sensitive. The role may also include development and delivery of mental health education sessions for the purpose of upskilling the CaFIS team members from both TeamHEALTH and Kalano Community Association.

The Senior Child and Family Wellbeing Practitioner will also work closely with the Team Leader to deliver strength-based case management and activities to children, young people and families in line with the Child and Family Wellbeing Service Program guidelines. Outreach support is also provided to assist in the development and implementation of participant centred Family Action Plans.

Reporting to and working closely with the Program Manager, the Coordinator will assist in the development, implementation and delivery of the Child and Family Wellbeing Service in the Palmerston/Litchfield region. They will be required to deliver strength based case management in line with program guidelines.

To facilitate the development of strong community relationships and support for participants of the CFWS program, the Coordinator will work with the Program Manger to develop collaborative partnerships and formal links with other services in the Palmerston/Litchfield region. They will assist in establishing good referral pathways, into and out of the service in order to reach vulnerable children, young people, families and carers, who may not otherwise engage with the mental health or children's services sector.

The Coordinator will play a key role in supporting, supervising and training the local Child and Family Wellbeing Service staff and make recommendations to the Program Manager on gaps identified in the service.

Child and Family Wellbeing Worker - Palmerston

Location: Palmerston

Reporting to the Coordinator the Child and Family Wellbeing Worker, will deliver strength-based case management and activities to children, young people and families in line with program guidelines. Outreach support is also provided to assist in the development and implementation of child centred Family Action Plans.

The Child and Family Wellbeing Worker will assist in establishing good referral pathways, into and out of the service in order to reach vulnerable children, young people and families who may not otherwise engage with the mental health or children’s service sector.

Under the direction of the Coordinator the Child and Family Wellbeing Worker will also provide education, training and information to promote awareness of mental health to participants of the program and the community.
