Recovery Assistance Program (RAP)

The Recovery Assistance Program (RAP), funded by the Northern Territory Department of Health, delivers targeted mental health support for individuals facing challenges that affect their ability to manage everyday life. Available in the Greater Darwin and Katherine Big Rivers Region, this service focuses on recovery planning and resilience-building.

What we offer

Whether you're at the start of your recovery or working toward greater independence, the Recovery Assistance Program can help you move forward with confidence and support tailored to your needs:

  • Individual Recovery Planning

Work one-on-one with a Mental Health Recovery Worker to set personal recovery goals and develop a plan that supports your emotional, mental, and social wellbeing.

  • Skill-Building for Daily Life

Develop practical skills to manage stress, improve communication, strengthen relationships, and handle daily responsibilities like budgeting, meal preparation, and time management.

  • Occupancy Support

Assistance with maintaining safe and stable housing—including help with paperwork, tenancy responsibilities, and linking with housing providers or advocacy services.

  • Navigation and Referral Support

Access help to connect with healthcare services, Centrelink, community support programs, and mental health specialists to meet your wider needs.

  • Strengthening Community Connection

Support to build confidence in engaging with others, attending groups, and reducing isolation through social inclusion activities and local supports.

Who is Eligible

The Recovery Assistance Program supports people who:

  • Are aged 16-64
  • Are living with a diagnosed mental illness or are experiencing ongoing mental health concerns
  • Reside in the Greater Darwin or Katherine Big Rivers Region
  • Would benefit from psychosocial support to build capacity, confidence and wellbeing
  • Do not currently received NDIS-funded support

I understand that this information will be stored on the TeamHEALTH system and that my details will be de-identified if they are used in reporting. I give permission for TeamHEALTH to discuss this information for the purposes of establishing and receiving supports.

Individuals are not required by law to provide this information or consent to this proposed use and disclosure of information.
