NDIS Supports

Supported Independent Living

TeamHEALTH’s Supported Independent Living (SIL) program provides 24-hour support to people with an NDIS plan in a safe and secure long-term home.

Our Mental Health Recovery Workers partner with residents to empower them to make informed decisions and to address matters that impact their ability to live independently in the community.

To participate in this program people must be over 18 years old and have an NDIS plan with appropriate funding.

Referrals are accepted from Local Area Coordinators (LAC), Support Coordinators, NDIA and self-referrals.

Short Term Accommodation (STA)

Short Term Accommodation (STA) offers a safe place for people to experience time away from home. This accommodation offers disability access and is tailored to the person’s specific needs.

People are welcome to stay for a minimum of 48 hours or a maximum of 14 days (this is assessed on a case by case basis). During this time TeamHEALTH will arrange for all the required care the person needs including:

  • Tailored community activities such as doctors’ appointments, family visits and outings related to NDIS plan goals.
  • Self-care including everyday living support such as cleaning, looking after their personal care and other everyday living support.
  • Opportunities to take part in social and skill building activities such as community outings, BBQs, cooking classes, fitness and wellness classes, group walks and day trips to local nature parks.

Short Term Accommodation (STA) is for people with an NDIS plan who need to change their location of residence for a short time or their carer’s, families or supports need a respite from their everyday support. NDIS participants need to have the appropriate core funding in their plan.
