Community Housing Program
The Community Housing Program (CHP) provides medium and long term housing options for people who have the capacity to live independently. Shared and non shared housing options are available. CHP tenants receive support from TEAMhealth Recovery Assistance Program (RAP). Referrals are accepted from any source.

- Be between 16-64 years of age
- Have a diagnosed mental illness
- Be allocated a Top End Mental Health Services Case Manage
- Be engaged with or be willing to engage in one of TEAMhealth outreach programs.
